ASCP Phlebotomy Certification

In order to be eligible for the Phlebotomy examination and earn your certification, (Phlebotomy Technician – PBT ASCP), you must meet the minimum requirements of one of the ASCP options below and pay the application fee which is currently $125.00 USD.

Certification Option 1: You must be a high school graduate (or GED/proficiency equivalent) and have successfully completed  a NAACLS approved phlebotomy course or a phlebotomy program that has been pre-approved  by The California Department of Health Services within the last 5 year span.

Certification Option 2: You must be a high school graduate (or GED/proficiency equivalent) and have successfully completed an approved two-part  phlebotomy program in the U.S., Canada or a CAP/The Joint Commission/AABB accredited laboratory within the last five years. You must have completed a minimum of 40 hours of classroom training, including human anatomy and circulatory system physiology, specimen collection protocols, proper specimen processing and handling and medical laboratory operations covering  quality control and safety. In addition, you must have completed a minimum of 100  hours of orientation and clinical training and in a CLIA regulated and accredited lab environment  with a minimum of 100 completed, unaided blood collections including venipunctures (vein insertion) and patient skin punctures.

Certification Option 3: You must be a high school graduate (or GED/proficiency equivalent) and have successfully completed 1 year of full time on-the-job work experience as a phlebotomy technician in a CLIA regulated and accredited lab environment within the last 5 years. Your job duties must have included successful venipunctures (vein insertion) and patient skin punctures. In order to be considered as a full-time employee, your job hours must be at least 35 hours per week.

Certification Option 4: You must be a high school graduate (or GED/proficiency equivalent) and be a licensed  RN/LPN or other accredited health professional which occupational training includes phlebotomy training, procedures and live performance and orientation in a CLIA regulated and accredited lab environment with a minimum of 100 completed and single blood sample collections including including venipunctures (vein insertion) and patient skin punctures. You must have a copy of your state or provincial RN or LPN or copy of your certificate of completion from the accredited health program you successfully completed (both would need to be notarized  before submission).

Certification Option 5: MLT/ASCP/MT/MLS/ASCP  Certification.

Certification Option 6: DPT/ASCP certification and 100 successful non-donor blood collections including successful venipunctures (vein insertion) and patient skin punctures under a CLIA regulated and accredited lab environment within the last 5 years.
